Example of machine lace picots in plaits. The loops are made with pairs, but they are untwisted, so separate.

Yes, I agree. Another case where one elements cannot be taken out of context. Seeing the whole thing, it is machine made.
Well, I'm tempted to agree with you. But the whole piece looks like this:
and it doesn't just have two thicknesses of thread - it has THREE! Various fine detail show non-standard thread movements, for example, the thickest thread weaves through some cloth stitch as a single thread rather than a pair. So the piece is machine made, I'm sure. That being so... I was assuming that a hand lacemaker wouldn't mind twisting a pair a few times, even if taking a short-cut to making a picot. But a machine might find it harder, although possible, I guess. Still, LOTS of twists might put too much burden on it, especially considering the amount of picots in this piece. That was my reasoning anyway. The piece as a whole is definitely machine made. Whether this type of picot is diagnostic of machine-made is perhaps more questionable. But none of the hand-made examples in my collection (and there are some shockingly bad picots!) are quite this bad.
Not certain they are machine made. Could be sloppy hand made.
